Your MDAS score
Your score is below the questionnaire's cut-off for high dental anxiety
Feeling a little nervous about dental visits is common. If anything about a visit worries you, you can tell the dental team beforehand and they will plan with you.
Your score is at or above the questionnaire's cut-off for high dental anxiety
Many people feel this way about dental care. Telling the dental team before your visit helps them plan a pace and an approach you are comfortable with.
The developers of the questionnaire treat 19 or more as a rough guide to a high level of dental anxiety, not as a fixed line.
This self-check is not a diagnosis. It cannot tell you whether you have a condition.
Sedation to help you relax
The clinic offers inhaled and oral sedation to help you relax. A dentist will assess whether it suits you.
How a dentist can help
Tell the dental team which part worries you most. The dentist assesses and suggests options that suit you.

How to read the result
Each question is scored from 1 (not anxious) to 5 (extremely anxious). Add the five scores for a total from 5 to 25. The developers of the questionnaire use a score of 19 or more to show a high level of dental anxiety.
This questionnaire does not diagnose anything. It is a way to put words to how you feel and to start a conversation with your dentist.
Privacy
This questionnaire runs in your browser. Your answers, height, weight and score are not stored and are not sent to analytics or advertising tools. Closing or reloading the page clears your answers. The only way your score reaches the clinic is if you tick the box to attach it to an appointment request below. Then one line with your score is added to your message, and only the clinic appointment staff see it.
