Dental anxiety self-check (MDAS)

Feeling nervous about the dentist is very common. These 5 questions, from the Modified Dental Anxiety Scale, help you see which parts of a dental visit worry you most, so you can tell the dental team what would help.

How to read the result

Each question is scored from 1 (not anxious) to 5 (extremely anxious). Add the five scores for a total from 5 to 25. The developers of the questionnaire use a score of 19 or more to show a high level of dental anxiety.

This questionnaire does not diagnose anything. It is a way to put words to how you feel and to start a conversation with your dentist.

Privacy

This questionnaire runs in your browser. Your answers, height, weight and score are not stored and are not sent to analytics or advertising tools. Closing or reloading the page clears your answers. The only way your score reaches the clinic is if you tick the box to attach it to an appointment request below. Then one line with your score is added to your message, and only the clinic appointment staff see it.
